I had a similar problem. with any stance I would lean forward. I would usually lean excessively on the ascent causing the hips to rise and do a kind of good morning type of squat. It is because of a disproportionately strong back with weak thighs i squatted this way. I would advise doing bodybuilding squats or leg presses. + if you squat with a wide stance maybe lower the weight and just work on keepin the back straight. Also it could be a balance issue if your a close stance squatter your knees hafta come past your toes. simmons recommends having shins perpindicular w/ floor but this is cause he squats wide, I dunno if you were trying this philosophy w/ the close stance. Another idea it could just be a flexibility issue. Maybe stretch after squatting.
