I have a 16 year old son who wrestles and has had 2 shoulders class 1 dislocations of the AC joint. The first time I took him to a our family doctor/ this was a major mistake. The second time I took him to a specialist. He noted that my son's chest and front deltoids were far more devloped than his back. So part of his re-hab was to work the upper back to balance the muscle groups. He went through physical therapy and has been healthy. In both cases when he injured his shoulder there was something in common. Both times he was wrestling someone was 20+ heavier than him and at the end of a tough practice. E-mail me and I can help you with some rehab exercises.
