Duration of bench pause from 60s to present

In his response to Steve Denison, Gene Mozee wrote of recommending a 2-second pause on the chest for the bench press. I'm curious about a couple of things:

1) Have any rulebooks ever prescribed a duration of motionlessness before giving a press command/clap?

2) How has the duration of the pause evolved from the 60s through today? Was there a "tipping point" where the duration changed significantly?

My impression is that the pause rule exists for the same reason one can't drop the bar after locking out a pull: to demonstrate mastery over the weight.
